Notice of Privacy Practices (HIPAA)

Effective October 6, 2026

THIS NOTICE DESCRIBES HOW HEALTH INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

MEC RESET LLC dba Menowave, an Idaho limited liability company registered to do business in Ohio, located at 5813 Monroe St. #127, Sylvania, Ohio 43560 (the "Company," "we," or "us"), is committed to protecting and promoting the rights of each of its clients. This Notice of Privacy Practices ("Notice") applies to all of our programs and services, including the Menopause Empowerment Circle ("MEC") coaching program and the Menowave Club membership ("The Club").

Our services are delivered by credentialed professionals, including a registered dietitian and licensed therapist. Although we do not bill health insurance, we follow the privacy standards of the Health Insurance Portability and Accountability Act (HIPAA) for the health information you share with us. This Notice explains how we may use and disclose your protected health information ("PHI"), your rights regarding that information, and our responsibilities.

A. Who Will Follow This Notice

This Notice describes the practices of:

  • Any health professional or coach authorized to enter information into your client record.
  • All programs and departments of the Company, including MEC and The Club.
  • All employees, staff, coaches, and other Company personnel, including Independent Contractors.

B. Our Pledge Regarding Your Health Information

We understand that information about your health is personal. We create a record of the coaching, nutrition, and wellness services you receive with us so we can provide quality care. This Notice applies to all records of your care generated by the Company. Your personal doctor or other providers may have different policies for the information they keep, so please ask them about their own practices.

We will:

  • Keep health information that identifies you private.
  • Give you this Notice of our duties and privacy practices.
  • Follow the terms of the Notice currently in effect.
  • Notify you promptly if a breach occurs that may have compromised the privacy or security of your information.

C. How We May Use and Disclose Your Health Information Without Your Authorization

For Care. We may use your PHI to provide coaching, nutrition, and wellness services. We may share it with Company coaches, health professionals, and Independent Contractors who are involved in your care, so your support is coordinated.

For Payment. We may use your PHI as needed to bill you and collect payment for our services. We do not bill insurance or send your information to health plans. If you ask us for a receipt or statement to submit to your own insurer or health savings account, we will provide it to you directly.

For Health Care Operations. We may use your PHI to run our programs and make sure clients receive quality care. For example, we may review our services, evaluate the performance of our team, or combine de-identified information about many clients to decide which services to add or change.

Appointment Reminders. We may contact you by email, text, phone, or voicemail to remind you of sessions, group calls, or check-ins.

Service Alternatives and Health-Related Services. We may tell you about other Company programs, services, or resources that may be of interest to you. You may tell us at any time that you no longer wish to receive this information.

Individuals Involved in Your Care. With your agreement, or if you do not object when given the chance, we may share relevant information with a family member or friend who is involved in your care or helps pay for it.

As Required by Law. We will disclose your PHI when required by federal, state, or local law.

To Avert a Serious Threat to Health or Safety. We may disclose your PHI when necessary to prevent a serious threat to your health and safety or that of another person or the public. Any disclosure will be only to someone able to help prevent the threat.

Surveys. We may use your PHI to ask about your satisfaction with our services.

Business Associates. We use outside companies to help deliver our services, such as video call, scheduling, community, file storage, and email platforms. When these companies handle your PHI, we take steps to require them to protect it.

D. Special Situations

Public Health and Safety. We may disclose your PHI for public health activities and to notify the appropriate government authority if we believe a client has been the victim of abuse, neglect, or domestic violence. We will make that disclosure only if you agree or when required or authorized by law.

Health Oversight Activities. We may disclose your PHI to a health oversight agency for activities authorized by law, such as audits, investigations, and the professional licensure process for our credentialed providers.

Lawsuits and Disputes. If you are involved in a lawsuit or legal dispute, we may disclose your PHI in response to a court or administrative order. We may also disclose it in response to a subpoena, discovery request, or other lawful process, but only after efforts have been made to tell you about the request or to obtain an order protecting the information.

Law Enforcement. We may release your PHI to a law enforcement official when required by a court order, subpoena, warrant, or similar process; to report possible criminal conduct at the Company; or in an emergency to report a crime.

Coroners and Medical Examiners. We may release your PHI to a coroner or medical examiner when needed to identify a deceased person or determine the cause of death.

E. Uses That Require Your Written Authorization

We will obtain your written authorization before we:

  • Use or disclose psychotherapy notes, except in the limited cases the law allows.
  • Use or disclose your PHI for marketing purposes, including your photo, full name, or identifiable story. De-identified wins may be shared as described in our Terms of Use.
  • Sell your PHI. We do not sell client information.
  • Make any other use or disclosure not described in this Notice.

You may revoke an authorization in writing at any time. If you do, we will stop using or disclosing your PHI for the reasons covered by that authorization. We cannot take back disclosures already made with your permission, and we must keep our records of the care we provided to you.

F. Group Programs, The Club, and Recordings

MEC and The Club include group coaching calls and an online community. Information you choose to share in a group call, post, or comment can be seen by other members. It is not part of your private client record, and we cannot control what other members do with it. Please share in group settings only what you are comfortable having other members know.

Members agree to keep what others share private and not to record, screenshot, or repost other members' information.

We may record group calls and expert sessions so members can watch them later. Recordings are shared only inside the program or The Club. One-on-one sessions are recorded only with your permission. You can protect your privacy on group calls by turning off your camera, changing your display name, or using private chat.

G. Your Rights Regarding Your Health Information

To use any of these rights, send your request in writing to [email protected].

Right to Inspect and Copy. You may inspect and get a copy of the PHI we use to make decisions about your care, including billing records, in electronic or paper form. We will respond within 30 days and may charge a reasonable, cost-based fee for copies. In limited cases we may deny your request, and you may ask that the denial be reviewed by another licensed professional chosen by the Company who was not involved in the original decision.

Right to Amend. If you believe information in your record is incorrect or incomplete, you may ask us to amend it, along with your reason. We may deny the request if the information was not created by us, is not part of your record, is not information you may inspect, or is accurate and complete.

Right to an Accounting of Disclosures. You may request a list of certain disclosures we have made of your PHI for up to six years before your request. The first list in any 12-month period is free. We will tell you the cost of any additional lists in advance.

Right to Request Restrictions. You may ask us to limit how we use or share your PHI for care, payment, or operations, or with family or friends involved in your care. Tell us what information to limit, whether to limit use, disclosure, or both, and to whom the limit applies. We are not required to agree, but if we do, we will honor it unless the information is needed in an emergency.

Right to Confidential Communications. You may ask us to contact you in a certain way or at a certain place, such as only by email or at a specific address. We will not ask why and will honor all reasonable requests.

Right to Be Notified of a Breach. You have the right to be notified if a breach compromises the privacy of your health information.

Right to a Paper Copy of This Notice. You may ask for a paper copy of this Notice at any time, even if you agreed to receive it electronically.

H. Changes to This Notice

We may change this Notice at any time, and the changes may apply to information we already have as well as information we receive in the future. We will post the current Notice on our website with its effective date, and give you a copy on request.
